Marquette Programs
Great Lakes Recovery Centers is looking for a part-time (aprox 16-24 hrs/week) In-Home Case Manager for the Marquette, MI Programs. Needs to be organized, be able to work independently, have reliable transportation, and enjoy working with people

The In Home Care Coordinator will be responsible for coordinating aspects of services related to the In Home Treatment Team (IHTT). The IHTT delivers substance abuse related services to clients in their home. The In Home Care Coordinator is responsible for organizing and ensuring that those services are delivered to our clientele. This includes, but is not limited to, keeping client records up to date, completion of paperwork duties, and assisting with tracking clients from discharge through aftercare programming. Additional duties to include assisting with referrals, coordinating client appointments and handling client dialogue between outside agencies and workers.

Minimum requirements include a Bachelor Degree in Human Services or related field. Degree with 1-2 years’ experience providing services in a counseling or human services related role. Additionally, ability to facilitate meetings and work on a timeline is a requirement. Previous Case Management experience helpful. Characteristics include strong written and oral communication abilities, excellent case conceptualization skills, strong organizational qualities, and strong leadership and team participation required.
